  • Guardian Name and Contact Information

  • Physical address, including city and state

  • Language spoken

  • Gender

  • Sexual orientation

  • Goals

  • Messages Sent

  • Program evaluations and survey data

  • Video recordings of mentoring sessions

How We Use the Personal Information Collected

The Sam & Devorah Foundation collects, retains, and processes – for as long as is reasonably necessary to satisfy the purpose of its collection – such information as is needed to

  • Deliver the services, mentoring, and programming provided by the Sam & Devorah Foundation;

  • Properly pair mentees and mentors;

  • Monitor and oversee the mentoring program;

  • Support community-based fundraising and campaigning;

The Sam & Devorah Foundation is the sole owner of the information collected on this website.  Our mentoring program is currently run through a licensed platform provided by Mentor City.  In the course of using or participating in the Sam & Devorah Foundation programs and services, you may provide Personal Data through the Mentor City platform or to us directly.  Mentor City will only collect, maintain, and use your Personal Data to the extent it is necessary to provide the services and programming offered through the Sam & Devorah Foundation.  Because our privacy policy does not apply to third party services or the practices of companies that we do not own or control, including other companies you might interact with through our website, we urge you to also review the Mentor City privacy policy https://www.mentorcity.com/en/legal/privacy-policy.

Changes to Personal Information can be made by contacting info@samdevorah.org.  If you would like your information to be deleted from our systems, you must contact info@samdevorah.org.  However, we reserve the right to maintain and aggregate your data in ways that anonymize it for ongoing studies and research purposes.

Children’s Privacy on this Website 

This website is not intended for use by children under the age of 13. We do not knowingly collect Personal Information from anyone under 13 years of age through this website.  If you are under 13, please do not submit any Personal Information on this website.
